As stated in Section 7 of Act 696 relating to the Application to be included in the Witness Protection Program which clearly states that:

1. Any witness can apply to the Director General to be included in the Program.

           The form that needs to be filled is Borang Permohonan Melalui Saksi Sendiri : Borang 7(1)

2. An enforcement agency may, with the written consent of the witness, apply to the Director General for the witness to be included in the Program.

           The form that needs to be filled is Borang Permohonan Melalui Agensi / Jabatan : Borang 7(2)

3. If the witness is under the age of eighteen, the parent or guardian of the witness may apply on his behalf to be included in the Program.

            The form that needs to be filled is Borang Permohonan Melalui Ibu / bapa / Penjaga : Borang 7(3)
